This site reflects our commitment to encourage communities to understand and improve their own care systems for elderly and disabled persons.

Community leaders, including medical care providers, long-term services and supports providers, citizen groups, civic leaders, and public health officials, can ask for certain data about their own community, or use this site to post their own data, allowing them to share progress and insights quickly and inexpensively. In our experience, having community data on elderly or disabled people is powerfully helpful in encouraging a sense of responsibility for enabling actions to improve supply or quality of services to these populations, and for the care system as a whole.
